(Birmingham, AL) TheraNest by Ensora Health, a practice management and EHR platform built for mental and behavioral health therapists, has expanded its AI Session Assistant to support in-person sessions, bringing HIPAA-compliant AI therapy notes to therapists who see clients face-to-face, not just via telehealth.

The expansion addresses one of the most consistent requests from the therapy community: a documentation tool that works regardless of how a therapist sees clients. AI Session Assistant is built natively inside TheraNest, with no separate app, no third-party scribe software, and no new workflow to learn.

“AI In-Person Notes has been very good, very easy, and seamless compared to my previous methods of doing notes. It has given me so much extra time and energy. I feel more creative, able to spend more quality time with my family, and able to do things I love rather than spending as much time on documentation.”

Mandy Miller, Licensed Therapist

The documentation burden

Documentation has become one of the most significant contributors to therapist burnout, and the data backs it up. According to Ensora Health’s 2025 Future of Therapy Report, which surveyed more than 1,300 therapists nationwide, 82% report experiencing burnout or serious fatigue. Of those, nearly a third deal with it several times a week or every day. More than half (52%) point directly to administrative complexity as a primary driver.

The toll goes beyond the individual therapist. When documentation pressure follows practitioners into the session room, it splits their attention. Presence is the foundation of effective therapy, yet it quietly erodes under the weight of notes still waiting to be written.

AI Session Assistant was designed to close that gap. The tool listens during a session, then generates a clinically aware draft note when the session ends. The therapist reviews it, makes any edits, and signs off, a process most users report takes 10 to 15 minutes at the end of the day.

“What’s most valuable for me is that I can get to the end of a workday, spend 10 or 15 minutes reading through, making a couple edits, signing, and I can be done with my notes. I can be present, and then outside of work, I can be present too, and not be thinking about playing catch up.”

How it works

AI Session Assistant operates entirely within TheraNest’s existing platform: no separate login, no switching between tools, no additional integration required. When a session begins, whether in-person or via telehealth, the tool listens and generates a draft note in the background. At the end of the session, a structured draft is ready for the therapist to review, edit, and sign. The therapist stays present and engaged with the client throughout.

What’s included

AI Session Assistant includes three capabilities:

  • In-Person Notes (new): a draft note is ready at the end of each in-person appointment, eliminating the need to write or dictate notes after sessions.
  • Telehealth Notes: the same AI documentation experience for virtual appointments. Requires the TheraNest Telehealth add-on.
  • AI Case Summaries: surface a client’s full history before each appointment, including previous sessions, diagnoses, and treatment notes, so therapists can prepare in under 60 seconds.

Built with privacy at the center

Privacy is the most commonly cited concern among therapists considering AI tools; 79% of respondents in the 2025 Future of Therapy Report named it as their top hesitation.

TheraNest built AI Session Assistant to address that directly, with full HIPAA compliance built in from the ground up:

  • Client PHI is never used to train the AI model
  • Session recordings are automatically deleted after notes are generated

Built natively within TheraNest, the tool keeps all data inside the system. Therapists can confidently communicate this to clients.

Recognition and adoption

AI Session Assistant received a Gold Award at the 2026 Digital Health Awards, a national program recognizing innovation in health information technology. It is the only therapy notes software and AI scribe for therapists built natively inside a therapy-specific EHR.

Adoption trends in the broader therapy community reflect growing openness to AI documentation tools. According to the Future of Therapy Report, 40% of therapists already use at least one AI tool in their practice, with note-taking ranking as the second most common application. Looking ahead, 64% believe AI will have its greatest positive impact on administrative relief, and 61% expect meaningful improvement in documentation support.
