Audit-ready documentation checklist

Audit-ready documentation checklist
A chart-by-chart self-audit checklist for therapists, counselors, and group practices.
An audit request rarely comes with much notice, and when it does, your records have to speak for themselves. Payers look for specifics: session times, named interventions, current treatment plans, and notes that show why ongoing care is clinically indicated. This checklist gives you a repeatable way to catch documentation gaps before a payer does, whether you’re running a quarterly self-audit or onboarding a new clinician.
Use this checklist to:
- Pull a 5 to 10 chart sample per payer and work through every item
- Confirm each progress note names interventions and supports medical necessity
- Keep treatment plans current, signed, and tied to measurable goals
- Match CPT and diagnosis codes to what’s actually documented
- Cover telehealth requirements, from consent to client location for Medicare
- Stay ahead of prior authorizations, credentialing, and payer-specific rules
